1. Directions from I-15.
2. Take Exit 265 toward Provo Center Street.
3. Turn Left at N 500 W St.
4. Turn Right at W 940 N St.
5. Take the 1st Left onto N 300 W.
6. (Office is behind Utah Valley Regional Hospital).

Directions to the American Fork Office (1159 E. 200 N., #350):

1. I-15 North toward Salt Lake.
2. Take Exit 276, 500 East.
3. Merge onto 500 E toward American Fork.
4. Make a slight Right onto E State Rd/US-89.
5. Turn Left onto 4650 W (4650 W becomes 1100E).
6. End at 1159 E 200 N (Copper Peak Medical Bldg.).

Directions to the Payson Office (1000 E. 15 S., Ste. 350 ):

1. I-15 South toward Las Vegas.
2. Take Exit 250 toward Payson.
3. Turn Left onto N Main Street.
4. At first stop light turn Left onto UT-198/State Rd.
5. Keep going until you get to the hospital (building is attached to the hospital).
